CHRONIC DISEASE NURSE REQUIRED - NOOSAVILLE
Noosaville Seven Day Medical Centre is seeking an experienced RN/EEN for an immediate start and a minimum of 32 hours per week to manage and drive the CDM program at the clinic. This will include significant contribution to completion of GPMPs, Team Care Arrangements and Health Assessments (75+, 45-49yr, Healthy Kids) and utilising the patient database to identify eligible patients. You will work very close with the Practice Manager and the GPs to ensure an effective process, however significant autonomy will be provided. Assistance in the Treatment Room may also be required on occasions. Friendly, professional and busy team environment with a highly competitive pay structure.
