CARE COORDINATOR

Then come and join the Sunshine Coast Division of General Practice’s Indigenous Health Team to help create a better journey through the health system for Aboriginal and/or Torres Strait Islander People.
We are looking to fill two positions, one in the northern end of the Sunshine Coast (Gympie) and one in the southern area (Caloundra), with an immediate start for the successful applicants.
The positions are for a minimum of 24 hours per week and up to a maximum of 32 hours per week.
It would be an advantage if you have a strong interest in Aboriginal and/or Torres Strait Islander health; completed cultural awareness training; and have an understanding of the PIP Indigenous Health Incentive.
The position includes:

Care coordination of Aboriginal and/or Torres Strait Islander patients diagnosed with chronic disease(s)

Facilitation and provision of appropriate multidisciplinary care for Aboriginal and/or Torres Strait Islander People, who have been diagnosed with chronic disease(s), by working collaboratively with patients and Primary Health Care Services

Assisting patients to manage their health in a way that meets their individual needs and will result in improved health outcomes.
