13 HEALTH

COACH PROGRAM
Using 13 HEALTH telephone infrastructure, the Health Contact Centre implemented the evidence-based COACH Program® in February 2009. Projected participation target is 3000 patients per year. COACH is proven to reduce the risk of hospital re-admissions in cardiovascular patients by helping them manage their risk factors. Referral is made by clinical staff on discharge from a public hospital.Qualified health professionals call the patient every 4-6 weeks, over a 6-12 month period. Information and education specific to the patient's underlying risk factors, health targets and treatment is discussed. An introductory letter and progress updates are sent to the patient’s GP.
