MBS Item Number 900
A Home Medicines Review is for patients living in the community where the GP assesses a patient's medication management needs and following that assessment refers the patient to a community pharmacist for a review of their medications.
The GP must:
-
obtain the patient's consent
-
provide relevant clinical information into the review;
-
discuss the results of the review with the pharmacist
-
write a medication management plan following discussion with the patient
Criteria for use:
-
patient must be living in the community (patients in residential aged care centres receive a RMMR - Item 903)
-
requested by the patient's usual GP
-
patients who are at risk of medication misadventure as a result of:
-
their co-morbidites
-
age
-
social circumstances
-
the characteristics of their medicines
-
the complexity of their treatment regimen
-
their lack of skills and or knowledge to use medicines to their best effect
-
Patients that may be suitable are those:
- currently taking five or more regular medications
- taking >12 doses daily
- significant changes made to their medications in the last three months
- on medications requiring monitoring or with a narrow therapeutic index
- symptoms suggestive of an adverse drug reaction
- sub optimal response to treatment
- suspected non compliance or inability to manage their medicines
- recent discharge from a facility or hospital within the last four weeks
