Palliative Care

The Rural Palliative Care Project (2010-2011) is being implemented through the Sunshine Coast Division of General Practice (SCDGP), coordinated by the Australian General Practice Network and funded through the Department of Health and Ageing (DoHA). 
 
AIMS OF THE PROJECT
This project encourages innovative practice to meet the challenges in delivering excellence in palliative care in rural settings. There is an important focus on enhancing partnerships and creating linkages between the services coordinating this care.  The project aims to ensure that rural Australians living with a terminal illness and their families have access to timely and quality palliative care services in the environment of their choice.
 
IMPLEMENTATION OF THE PROJECT
SCDGP has determined that the settings of care focus, for this project, include General Practices and Residential Aged Care Facilities in the designated rural area and all Private Hospitals within the Division boundaries.  Gail Palmer has been appointed as the Palliative Care Resource Nurse.  This is not a clinical role but aims to provide resources that will promote communication and coordination of care between service providers.
