The Rural Palliative Care Project (August 2008 - April 2011) was implemented through the Sunshine Coast Division of General Practice (SCDGP), coordinated by the Australian General Practice Network and funded through the Department of Health and Ageing (DoHA).
This project encouraged innovative practice to meet the challenges in delivering excellence in palliative care. It focussed on enhancing partnerships and creating linkages between the services coordinating this care.
The content of the palliative care resources pages on this website was collated as a major focus of the Rural Palliative Care Project. The information was collected in collaboration with expert health care providers. It is intended to support health care professionals and patients at the end of their lives and their families. We encourage you to use this information to ensure that Australians living with a terminal illness and their families have access to timely and quality palliative care services in the environment of their choice.
The following pages are organised to provide quick and efficient links to:
- General Practice Support
- Services Directory
- Specialist Information
- Clinical Guidelines
- Information for Patients, Families and Carers
- Useful Links
