PREVENTIVE CARE (LIFESCRIPTS)

This initiative has been developed to support the SNAP framework and to assist GPs in facilitating the management of patients who are seeking to make lifestyle choices and changes. The development of resources has been funded by the Department of Health and Ageing (DoHA).

The resource kit contains a number of resources including a poster for the waiting room, patient information and prescriptions for noting lifestyle changes.
