Overview

Proactive Primary Care (PPC) is the name given to the ongoing strategy that the Sunshine Coast Division of General Practice is carrying out to assist general practice in the areas of prevention and improving chronic disease management.
The current delivery is through a number of simple clear workbooks on a range of practice interests and needs. Practices can choose to do one workbook or all of them. They are self-paced and provide easy access to resources and education.
 
The books have a simple, clear design and plans/actions that are not difficult to implement and are designed to work within the GP environment and do not impact heavily on practices time and resources.
 
Impressive patient outcomes can be reached by simply interrogating your practice data, keeping data up to date and clean, improving efficiencies and building a stronger practice team through Inter-Professional Learning (IPL) principles.
 
IPL takes place when two or more professionals come together for educational purposes, there are opportunities to learn with and from each other. The Sunshine Coast Division of General Practice (SCDGP) provides both formal and informal education that caters to the whole practice team. Informal learning opportunities also arise from the practice team teaching and learning from one another.
 
Ultimately the plans/actions work toward streamlined systems and efficiencies that contribute to easier management of your patients. 
 
Successful outcomes include better managed patients with improved health outcomes (and return on effort that demonstrates sustainability).
 
At the end of each workbook there is a maintenance schedule that should be repeated periodically to ensure that the data quality remains high, ensuring outlaid successes are not lost.
 
As the disease specific workbooks act as a clinical audit, upon successful completion of each workbook, RACGP have tentatively approved 30 Cat 1 CPD/ CNE points for the 2010-2013 triennium for participating GPs and Nurses. There will be opportunities for recognised prior learning for practice staff participating in the Certificate III in health administration through SCDGP in 2011.
 
At the end of each module you will receive a printed log of quality improvement evidence that you can use to demonstrate your quality improvement activities for your accreditation as per RACGP standard 3.1.1 Quality Improvement Activities.
